Annex to the Data Processing Agreement between OPTISWIFT S.R.L. and the Practice, published at https://www.optiswift.ro/abonament/contract-imputernicire, in the version the Practice accepts together with this annex; the annex forms an integral part of the Agreement (Clause 1(d) of the Agreement). This annex is published at https://www.optiswift.ro/abonament/anexa-i and is accepted at the “Contract” step, together with the Agreement: when the subscription is activated, when online payment is set up, when the subscription is resumed or when a new version is accepted, in the Software or at www.optiswift.ro/cont. Capitalised terms (“the Provider”, “the Practice”, “the Software”, “the Service”) have the meaning given in the Agreement.
The controller
The controller is the company that enters into the OptiSwift subscription contract (the Practice), identified by the details declared on activation or on accepting the documents in the Software or at www.optiswift.ro/cont. The Provider records these details together with the acceptance of this annex, and they complete this annex for that Practice:

The Practice updates its email addresses in the Software (Settings → Account and Settings → Subscription) or at www.optiswift.ro/cont; it notifies the Provider of any other change to the details above at contact@optiswift.ro.

The Provider’s agreement follows from the publication of this version and from the confirmation the Provider sends the Practice after each acceptance (the Agreement, § 11.7). The Provider updates its contact details in this annex by a communication sent to the Practice by email (the Agreement, § 11.9 and § 11.10).
Entering into the Agreement on paper or with a qualified electronic signature
A Practice that enters into the Agreement on paper or with a qualified electronic signature completes and signs Annex I in the text of the Agreement, and the signed copy prevails (the Agreement, § 11.7).
